The viral Ozempic before and after photo hides one thing: it is the end of months of a calorie deficit, not an overnight switch.
Semaglutide and tirzepatide cut hunger for real. But the "after" you see follows a predictable timeline, with slow stretches and fast ones.
Here is how much you can lose at 3, 6, and 12 months, why the scale barely moves at first, and what makes the result last.
What the before and after photo actually shows
The visible result is months of a calorie deficit that the drug made easier to keep, not an automatic effect of the needle.
A weight loss injection works on appetite. It mimics the GLP-1 hormone, which tells the brain you are full and slows how fast the stomach empties.
With less hunger, you eat less without a fight. In the STEP 1 trial, published in the New England Journal of Medicine, people on semaglutide 2.4 mg (Wegovy) lost an average of 14.9% of their weight in 68 weeks, versus 2.4% on placebo.
Notice the word "weeks." The dramatic before and after belongs to someone who stayed in a deficit for months, not someone who took one dose.
In numbers: semaglutide leads to about 15% weight loss in a little over a year. Tirzepatide reaches close to 21% at the 15 mg dose. These are study averages, at full dose and with medical follow-up.
Ozempic before and after: the 3, 6, and 12 month timeline
Most of the weight comes off between month three and month twelve, once you reach the full dose and appetite stays low in a steady way.
The first weeks use a low dose on purpose. It lets your gut adjust, it does not melt fat.
| Time on treatment | Semaglutide (Ozempic, Wegovy) | Tirzepatide (Mounjaro, Zepbound) |
|---|---|---|
| 3 months | 4% to 6% of weight | 6% to 9% of weight |
| 6 months | 8% to 10% | 12% to 15% |
| 12 months | around 15% | up to 21% |
For someone at 200 lb, 15% is about 30 lb in a year. On high-dose tirzepatide, the number can pass 40 lb.

Track your protein while the weight comes off
When appetite drops, it is easy to eat too little and lose muscle along with fat. Snap your meal and see calories and protein instantly.

Why the scale barely moves in the first weeks
In the first month the dose is still adjusting, the drug takes four to five weeks to reach a steady level in the blood, and part of the early loss is water.
Almost every protocol starts at 0.25 mg per week. That is a training dose for your gut, not a fat-loss dose.
This is the part of the before and after no one posts. The real progress photo only starts to change after the second month.
Important: these drugs are prescription only. Dose, titration, and follow-up are your doctor's call. Compounded pens or self-dosing carry real risk of side effects and of buying a product with no guarantee.
Semaglutide, tirzepatide, or liraglutide: the "after" changes with the molecule
The newer the molecule, the larger the average loss tends to be, which is why two before and afters over the same time can look very different.
Liraglutide (Saxenda) is the oldest and leads to about 8% loss. Semaglutide lands near 15%. Tirzepatide works on two hormones and reaches 21% at the high dose, per the SURMOUNT-1 trial.
So the same "12 months" gives different numbers depending on what you inject. To compare each active ingredient, see the guide on semaglutide vs tirzepatide.
Dose, price, and tolerance matter too. The strongest molecule on paper is not always the best fit, and that call belongs to your doctor.
What happens to the "after" when you stop
Without a change in habits, stopping the pen usually brings part of the weight back, because hunger returns to its old level.
The STEP 1 extension followed people who stopped semaglutide. Much of the lost weight came back in the months after they quit.
That does not mean the drug failed. The result holds as long as the deficit holds, with or without medication.
If results stall before you stop, the cause may be different. If you are still weighing access, see how to get Mounjaro.
How to make your before and after last
The result becomes permanent when your plate holds the deficit on its own, not only through the reduced hunger from the shot.
The pen makes a deficit easier, but it does not guarantee one. You can blow past your target with no hunger at all, just calorie-dense portions.
This is where measuring helps. ContaCal is an app that estimates the calories on your plate from a single photo, using artificial intelligence.
You shoot the meal and see right away if it fits your daily target. Getting enough protein per day protects muscle while you lose.
Over time, you learn to build plates that hold the weight on their own. That is the "after" that stays after treatment ends.
